About the Customer

Choices Network of Arizona was a community-based clinically integrated network serving individuals experiencing serious mental illness (SMI) in Phoenix, AZ.  Choices Network operated six fully integrated health homes, serving 7,000 individuals determined SMI; including 2,000 individuals under court ordered treatment. 

With a focus on empowerment and education, Choices Network of Arizona aims to help clients make informed decisions and navigate through difficult situations with confidence.

The Challenge

Hess III Consulting was hired to manage the dissolution of a $40 million clinically integrated network that became insolvent; while ensuring continuity of care for 7,000 individuals with serious mental illness. Of these, 2,000 were under court-ordered treatment and needed their care transition to be authorized by the mental health court prior to transitioning them to new providers. The clinically integrated network was to be split between two organizations that agreed to the asset purchase and client transition. 

Our Approach

Hess III Consulting assigned a certified project management professional (PMP) with over a decade of specialty behavioral health experience. The project manager met Choices Network's executive leadership and with both acquiring entities collectively and individually weekly to protect their respective intellectual property. We applied best practices to complete due diligence, including inventory and valuation of all corporate assets, individually engaging individuals in care to notify them of the transition, coordination with the County mental health court to transition 2,000 individuals on court ordered treatment. We additionally liaised with the managed care organization and State Medicaid Authority providing patient-level updates until every individual was connected to care at their new clinic; which took over nine-months to complete.

Outcomes

The project achieved all its objectives within nine-months. Hess III Consulting's project manager successfully transitioned the assets and operations of the CIN to two separate providers, ensuring no disruption in patient care.
